only)
This
form provides parent/guardian permission for a student to leave the
school grounds during their lunch period.
This privilege is currently only available for students in 10th through
12th grades.

Athletic
Contract
These forms are required to be completed and returned
in order to participate in school athletics.
Emergency Procedure Locator Card
The
information on this card helps us to locate a parent/guardian/relative
in case of an emergency.
Physical Examination Form
This
form is completed and signed by the health provider who performs your
student's physical.
Sports Participation Parental Consent Form
The parent/guardian must complete parts A & B then return this form
to the school nurse two weeks prior to a sports physical. The physicals
are performed by a doctor or physician assistant from the Deposit Family
Care Center. If you are taking your child to your own family physician,
please notify the school nurse prior to doing so.
Sports Physical Interview Form
The parent/guardian completes and signs this form just prior to EVERY
sport (unless a sports physical has been done within the last 30 days).
